Question Type: Comment / Essay, Textboxes and Descriptive Text

Comment / Essay Box

Start by Selecting a Type of Question. Then, choose Comment/ Essay Box

Enter the Question text.

The Comment/ Essay Box Question Type is an excellent choice if you need to enter a narrative question such as a patient’s chief complaints in their own words.

For this Question you no longer need to add a comment box because this is already a comment box question.

When you are done, just click on Add Question.

This is what the Comment Box will look like.

You may enter as many lines as you would like.

Comment / Essay Box

Start by Selecting a Type of Question. Then, choose Comment/ Essay Box

Single Text Box

You may also choose Single Text Box if you wish to enter narrative information but it is a short answer.

Select Single Text Box from the Select a Question Type dropdown.

For this example I will choose Patient Age.

As you can see the textbox for this section is significantly shorter from the Comment/Essay box, making it a perfect choice for short answers such as patient age or name.

So I have less room to enter my answers.

To populate this textboxes you may choose Multiple Textboxes or Numerical textboxes

Multiple Textboxes

As you can see when you choose Multiple Textboxes, you will be prompted to enter in an Input Box choices.

Simply enter in your question options and click add for each one.

Below Input Box Choices you will see all the previously entered Question Options.

When you are done entering Question Options, click on Add Question.

Notice that you will have a selection of smaller textboxes where you can enter information under one question.

Single Text Box

You may also choose Single Text Box if you wish to enter narrative information but it is a short answer.

Select Single Text Box from the Select a Question Type dropdown.

Numerical Textboxes

You can use Numerical Textboxes if the answer to your questions are numbers

This is how it will appear.

Descriptive Text

The Descriptive Text question type is not really a question at all.

It it use for instructional purposes.

You will NOT be allowed to input any question options into this question type.

You will NOT have any options to choose from when you are actually using this Descriptive Text in your documentation.

Simply enter in the description Text and click add question.

As you can see, the Descriptive Text is not actually a question at all but it is simply use for instructional purposes.
